Image Challenge Q: What is the diagnosis in this 42-year-old man with bone pain? Answer: Gaucher's disease Bone involvement is common in Gaucher's disease. Flaring of the distal femur, the so-called Erlenmeyerflask deformity, is a classic sign of the disease. Marrow infarctions are also present throughout. Secondary osteoarthritic changes are due to subchondral bony collapse resulting from osteonecrosis.
