Severe mitral regurgitation was caused by malfunction of this mechanical prosthetic valve (Bjork–Shiley). The strut fracture of the prosthetic valve led to dislodgment of the occluding disc leaving the mitral valve wide open (A) associated with severe mitral regurgitation illustrated by color Doppler study (B).
