Four Steps To safety SCALING UP ACROSS ALL 67 INPATIENT SERVICE ACROSS BOTH ORGANISATIONS

Proactive Care DASA (Dynamic Assessment of Situational Risk) A tool used to predict risk of a violence Risk management Plans formulated by clinicians in response to identified risks of the patient. Zoning A system where a Red, Amber Green rating is used to identify the care needs for the service user with specific interventions delivered in relation to the zone rating.

Patient Engagement Compact The community code of conduct between patients and staff on the ward, which foster a therapeutic atmosphere enhancing the development of community wellbeing. Intentional Rounding Involves health professionals regularly engaging with patients and proactively meeting their needs .

Multi Disciplinary Team Working Report Out A morning meeting with the multi-professional team to discuss service user care. SBARD (situation, background, assessment, response, decision) A structured method for communicating clinical information which improves communication in the healthcare setting. Escalating Risk A set of standardised identified steps to take when the service user required support when in a condition of deterioration.

Environment Safe Wards Len Bowers Safewards Model included in the programme to further improve safety on the ward. Mapping of violent incidents Provides useful information about where incidents most commonly happen on the ward. Staff are then better able to target interventions to reduce incidents.
