Knowledge of the person, his/her situation and history through shared experiences (error, repeatable and not / risk )

Times and places for regular, periodic encounters within the working group concerning the patient situation’s knowledge gained over time and from different sources and based on staff members’ different experiences and abilities. Organising ourselves to work together: Build up the Team!

Seeking out and contacting for their possible involvement, all conceivable actors (institutional and not) who had, have or could play a role: in the person’s past, present and future life.

Seek, gather, convert, activate, negotiate the necessary resources. Create “ new social spaces”.

Times and places for regular, periodic encounters within the working group concerning the patient situation’s knowledge gained over time and from different sources and based on staff members’ different experiences and abilities. Afterwards, we can see at this project as the history of the relationships user/service/context.
