Phasing Example: Victorian participant phasing plan for North East Melbourne The transition to the NDIS in Victoria will occur based on activities currently funded by the Victorian Government. This is specific to the North East Melbourne Area, a unique phasing plan will be developed for each area prior to their scheduled transition. It is likely to be a similar rollout in each area For image on slide * Other disability activities include: community and facility based respite, flexible support packages, outreach, behaviour intervention services, case management, recreation, therapy and independent living training. ** MHCSS activities to transition to the NDIS are: Adult Residential Rehabilitation Services, Individualised Client Support Packages and Supported Accommodation Services. ^ Multi-Purpose Transport Program users will transition to the NDIS along with any existing disability, education or health related activities following the phasing in of all other first named programs.

Partnership approach The Victorian Government, Commonwealth Government and National Disability Insurance Agency (NDIA) are committed to working together to implement the NDIS.

Transition of people with a disability to the NDIS Supporting Access: Follow up process: supporting people to make telephone contact with the NDIA access team or to complete and return the NDIS access request form Warm transfer: directly transferring clients to a dedicated phone line within the NDIS access team. Delegated access: Vic Gov staff provided with delegated access to the NDIA Access tool.

More intensive intervention Supporting Access: Supported Access team. Focusses on accessing and supporting the ‘hard to reach’ group to enter the NDIS. Intensive Support team. Three key focus areas: Transition planning Shared participant planning Individual interventionist support

Other supports Pathways: Victoria and the NDIA are reviewing participant pathway options for people with complex needs, such as matching the planner’s expertise with participants needs. Workforce: The transfer of existing Victorian knowledge to the NDIA has been maximised, including the release of experienced Victorian Government staff to the NDIA to assist with: early transition of people on the Disability Support Register early Childhood Intervention Service waitlist applications planning with participants in transitioning areas. Supporting Readiness: Over $20 million in Transition Support Package funding has been invested to prepare participants and providers for the NDIS.
