Recommendation Although hazard hunts were conducted by key personal it is recommended by the team to; set a schedule and involve the crew members in future hazard hunts, engage crew by using probing questions (i.e. what could go wrong?, how can we make it safe?), encourage people to think and motivate them to bring forward solutions. need to allow quality time for safety dialogue on hazards and controls (highly motivated and knowledgeable staff and crew). Include hazard hunt in Contract HSE plan with the involvement of Petrogas and Medco management team. PDO CR requested support from MSE to verify and check the hazard hunt quality run by Petrogas and MEDCO during 2018.
