RFA Requirements Applicant Information Sheet: Include all services for which funding is requested. Units of Service and Targeting Projections: Include all services for which funding is requested. Program Narrative – Part 1: Submit a separate Program Narrative for each service requested Program Narrative – Part 2: Program Assurances signed by authorized representative. The name of the authorized representative may be typed if e- mailed. Applicant Checklist

Subcontractor FY18 Requirements EIN # Award Amount Funding Source Consumer contributions Required Match (Exhibit A) 25% Timely Submission (Exhibit B) By the 10th of the month following service provision request for reimbursement, service roster, and consumer intake forms Elderbridge may reduce total funds by a 2% penalty for late invoices Consumer intake forms required for: Homemaker, Material Aid, Outreach, Chore, Personal Care, Transportation, Respite, Health Promotion HIPAA Requirements (Exhibit C)

Unmet Needs Reporting THIS IS VERY IMPORTANT !!! What is an unmet needs/gap in service? Service request/need that: No funding is available Funding inadequate to meet the full need Lack of available staff Report is on the bottom of the RFR and needs to be completed monthly

Other Important Information Contract award should be expended at 25% per quarter in order to avoid service reductions or terminations later in the FY. Exceptions may be considered if submitted in writing. Projected funding requests must be as close to actual as possible as amendments to shift funds between contracted services will not be allowed in FY18 State 4.25% budget reduction for current FY and FY18
