HUMAN FAILINGS

Human failings We are all susceptible to forgetfulness unjustified assumptions jumping to the wrong conclusion complacency wishful thinking, etc 33% of all accidents and incidents in 2017 apparently arose in this way

An example follows

Circumstances incomplete preparation for flight in the air on the ground

Preparation for flight-1 insecure tailplane inoperative elevator (??); fatality

Preparation for flight-2 16 flights with: airbrakes open unlocked canopy loose batteries/ballast tail dolly attached rudder pedals insecure P2 with untested new hearing aid tug and glider canopies misted after take-off

In the air 3 tug upsets including: 1 from glider pilot looking at fallen instrument 1 on trial lesson with airbrakes open 7 failed final glides, one on a trial lesson opened canopy intending vent ridge soaring, gliders missed by 30-50m

On the Ground-1 not looking: prop hit cable buggy driven across the landing area landing glider almost hit a member injured by glider wing prop hit cable dolly with one clip remaining broke in use

On the ground-2 car driven over wing (2) towed glider collisions (8) towed glider, brakes open, destroyed by fire (de)rigging, wing fell over (3) tug prop hit towing bar injury from engine start with fuselage in cradle cable drawn into winch hit glider used u/c lever for flap

Human failings forgetfulness unjustified assumptions jumping to the wrong conclusion complacency wishful thinking, etc No easy solutions. But member awareness can help. Please show these slides at your club.
