Supportive Service Demonstration for Elderly Households in HUD-Assisted Multifamily Housing – Holy Rosary

Overview

Supportive Services Demonstration Overview Approximately $15 million was made available nationwide

Demonstration Goal Test a housing-with-services model to support Aging in Place Delay or avoid transitions to nursing homes Positive impacts on health and well-being

Demonstration Goal increasing tenure in independent living reducing transitions to higher levels of care unnecessary or avoidable emergency department visits, hospitalizations, etc. Our primary goal is to support our residents to live as long and as safely as possible where they are.

Eligibility Requirements Eligible Multifamily -Assisted Housing Types Section II.A.1 Housing that is assisted under section 202 of the Housing Act of 1959 (12 U.S.C. 1701q) Includes 202 PRACS & 202 Direct Loans Other Housing such as Section 8, Section 221, 236, and some rental assistance programs

Eligibility Requirements Eligible Multifamily Housing Properties Section II.A.1 The Assisted multifamily properties must: Have 50 or more assisted units Campus counts may be combined Be designated, restricted or reserved for occupancy by elderly persons Statutory or owner-adopted May serve no more than 10% of nonelderly, near-elderly and/or disabled families

The Domus Corporation

Domus History Incorporated in 1995, the Domus Corporation is a not-for-profit corporation whose purpose is to develop housing in the four northern New Jersey counties of Hudson, Essex, Bergen and Union, and to provide a broad range of housing and housing-related support services to persons in need. Domus functions as the housing development corporation on all Archdiocesan permanent-housing initiatives.

Domus History For over twenty years, Domus has been providing services within the Archdiocese of Newark. Domus understands and successfully functions within the various rules and regulations, including those mandated by the local and State government, as well as the funding agencies. Our established relationships - which are the direct result of our reputation, with the counties and HUD - enable us to bring projects to occupancy in as little as 12 months.

Domus History Domus has developed affordable senior housing that was provided funding through HUD Federal Home Loans, DCA Balanced Housing, County of Bergen, and County of Hudson.

Catholic Charities Connection Domus receives financial and administrative support from Catholic Charities of the Archdiocese of Newark (CCAN). Catholic Charities is the non-sectarian, not-for-profit social service corporation of the Roman Catholic Archdiocese of Newark, New Jersey. One of New Jersey's oldest and largest social service agencies, CCAN helps individuals and families in need to acquire assistance and a greater sense of self-worth and dignity.

Safety and comfort. Residents should feel safe and secure. All safety concerns are promptly addressed and residents feel they have some collective control over their environment. Accessible services. Support services should help ensure stability and maximize self-sufficiency. Housing stability is a basic and primary goal. Empowerment and independence. Residents are involved in the management of the project. Participation in tenant councils are encouraged.

Holy Rosary Demonstration Grant Service Coordinator and the Wellness Nurse will be hired in September and participate in required training. Funds will cover the FT Service Coordinator, PT Wellness Nurse, and some ancillary costs
