Exploration Lateral Learning What happened: The grader pusher had reset the GPS equipment inside the cab of a grader and began descending to the ground. He started in the correct position holding onto the handrails on either side of the access ladder. On descending he turned his body to the left to place his foot on the tire instead of the third step. As a result his foot slipped off and fell onto the top edge of the grader blade and then to the ground sustaining minor injury. Why it happened: Failure to follow rules/procedures. Three points of contact was not maintained while descending the access ladder. The Blade was in the wrong position Third step was not in a good shape Learning Points: The grader blade shall be moved into a position that if a fall was to occur, no contact with the blade take place. Ensure that the steps and foot holds are sufficient, in good shape, and have a non-slip surface.
