0-19 Norfolk Healthy Child Programme Cambridgeshire Community Services NHS Trust: providing services across Cambridgeshire, Peterborough, Luton and Suffolk

Norfolk Healthy Child Programme aims Protect and promote the health and wellbeing of all children, young people and their families - with the greatest improvements seen for those in greatest need so that health inequalities are reduced. Achieve positive outcomes for the Service User through an integrated programme that is delivered by coordinated partnership working. Be simple to access, with a clear range of pathways and choices. Be liked, valued and trusted by its users, partners and staff.

Includes development of P2P programme with Children’s Centres Improving School Readiness Being 2 in Norfolk – Shared review (Health and Early Years Education) 3.5 year school readiness review – partnership with GP practices

Teenage Parents Support Pathway – September 2017 Locality model, led by FNP Specialist Practitioner, including support and consultation for partner agencies Locality multi agency training offer– Solihull Approach and Attachment Re-imagining services for 5-19 – September 2017 Key priorities; emotional wellbeing, sexual health, health literacy Points of transition; transition to school, transition to high school, transition to adulthood Targeting support and interventions New service developed for 16-19 year olds – consultation underway

Complex Needs pathway Service user led HCP annual contact with every child and young person Referral pathway to complex needs services Looked After Children Health Reviews - Under 5’s April 2016 – new service implemented. LAC HVs undertake reviews – single Norfolk process Training Programme – designated LAC nurse Quality and timeliness of assessments– 100%
