Erythropoietin (EPO) measurements in patients with various forms of erythrocytosis. When compared with the normal response, polycythemia vera patients present with below normal to absent serum EPO levels. Patients with polycythemia secondary to hypoxia or an erythropoietin-secreting tumor show high-normal to elevated levels. The response to phlebotomy is even more distinctive. Polycythemia vera patients essentially follow the normal curve, whereas patients with hypoxia show marked increases in serum erythropoietin levels even while the hemoglobin level is above normal. Patients with erythropoietin-secreting tumors generally show a suppressed response to phlebotomy since EPO production is autonomous.
