Situation Creating stockpile of fill material Rock Trucks dumping on top of pile Bulldozer shaping material to minimize footprint and waste – Operator acting as spotter for trucks. Just prior to raising load, rear tires on right side of box section of truck slipped off edge of path and box tipped onto side.

Incident Witness Statement: 1:17pm “I drove by a rock truck backing up a sand hill. Looked in the rear view mirror and the truck was rolling over onto its side. I was the first responder and watched the driver leave the cab of the truck. I spoke with him and he was ok and calm. I called 55555 and reported to security. I called my section head to report the incident. The fire truck was there immediately and we secured the scene. It was determined that there were no fluids leaking from the truck. I took photos with a company camera. Security arrived and took statements from driver and witness. Deep River Police arrived (approximately 45 minutes later), took statements and pictures. There were no injuries and the vehicle had not rolled over. The contractor decided to use his equipment on-site to right the vehicle.”

Immediate Corrective Action: A BEI Mechanic was consulted prior to moving the rock truck. A bulldozer was chained to the front of the rock truck. A hoe was used to rotate the truck box upright. The hoe held the truck in place as the driver drove forward. The maneuver was completed smoothly with no equipment damaged or employees put at risk. No danger to the environment was present.

Dumping Guideline Dumping Articulated rock trucks are designed so that the cab is independent of the box. They are designed this way to provide safety for the operator and the machine so that when dumping in soft ground conditions the box can topple over without taking the cab with it. Topple Over Guideline (WorksafeBC) If the box topples over and there is no speed involved, there is not usually any damage done. Ensure the following before attempting to right the box: The lift cylinders have not been damaged. The rubber blocks or the bolts and brackets that hold the under-carriage to the frame are not damaged or broken. All of the hydraulic and electrical lines are still intact between the tractor and trailer. As much material is cleaned out of the box as possible. The box lever is placed in the down position; make sure the box is pulled or pushed down tight to the frame. This will put the box guides around the frame and prevent the rear box hinges from being twisted as the box is pulled up. Use an excavator as it is the best machine when righting the box as the excavator bucket can be placed inside the box in order to pull the box upright. If a loader or similar piece of equipment is used, attach a pulling cable to the box lift point at the cab end of the box.
