Crooked Oak Middle School Spring Incentive Program 2018

What is it? The incentive program is system for Crooked Oak Middle School Students to earn points through a variety of methods. All students will be eligible for this program Those students receiving the required number of points will go on a field trip to Frontier City on May 3. Tickets & meal will be paid for by the school

Points can be accumulated between January 3- April 28 TIME FRAME Points can be accumulated between January 3- April 28

HOW MANY POINTS ARE REQUIRED? 115 is the maximum number of points achievable 85 points required to go on trip to Frontier City

HOW DO I GET POINTS? There are 7 categories in which students can achieve points TARDIES ABSENCES DISCIPLINE REFERRALS 9 WEEK AR GOALS REPORT CARDS Zero’s on class work Benchmarks

0-1ST tardy: 20 points 2nd tardy: 10 points 3rd tardy: 6 points tardies 0-1ST tardy: 20 points 2nd tardy: 10 points 3rd tardy: 6 points 4th tardy: 0 points

ABSENCES 0-1 absence: 20 points 2 absences: 10 points ALL ABENCES WILL COUNT EXCEPT FOR THOSE THAT HAVE AN ACCOMPANYING DOCTOR’S NOTE SCHOOL ACTVITIY ABSENCES WILL NOT COUNT 0-1 absence: 20 points 2 absences: 10 points 3 absences: 6 points 4 absences: 0 points

No referrals: 10 points 1 referral: 5 points 2 referrals: 0 points Discipline referrals No referrals: 10 points 1 referral: 5 points 2 referrals: 0 points

AR GOALS Student’s that achieve the 3rd nine week AR Goals will receive 10 Points

Points for not having zero’s on class work 20 points for no zero’s in all classroom work 15 points for one zero in all classroom work 10 points for two zero’s in all classroom work 5 points for three zero’s in all classroom work All faculty must report zero’s two days before posting dates to Robin No exceptions

Report cards Student’s that get no D’s or F’s on the third 9 weeks report cards will receive 15 points

BENCHMARKS 6-7-8 Grades Reading & Math Benchmarks 20 Points for Advanced 15 Points for Proficient

What happens if I get close to required points but fall a little short?? For all students that come within 10% of required points a Principal’s Advisory Council which consists of 6-7-8 faculty members will meet & decide on potential bonus points.

ALL POINT TOTALS ARE FINAL AND WILL NOT BE ADJUSTED IN ANY MANNER ALL DECISIONS MADE BY THE PRINCIPAL’S ADVISORY COMMITTEE ARE FINAL NO EXCEPTIONS

There will be a list of student’s with point totals posted in commons Posting Dates January 26 February 9 February 23 March 9 March 30 April 13 April 27
