Patient Centered Primary Care Collaborative Leadership Center for Multi-Stakeholder Demonstration ●Sally Bleeks, Sally Bleeks, Managing Director Office of Clinical Affairs, BCBSA ●Julie Schilz, IPIP and PCMH Manager Colorado Clinical Guidelines Collaborative ●Shari Erickson, Senior Associate, Practice Advocacy and Improvement American College of Physicians ●John Swanson, Director Practice Support Division, American Academy of Family Physicians ● Rohan Beesla, Director of Policy, ERIC Function: Identify community-based sites to test and evaluate the concept; share information and best practices about pilots within a collaborative community; and serve as the connector to technical, quality improvement and education resources to facilitate ongoing demonstrations.
