Why we’re doing this Patient voices aren’t typically included To get feedback from the community about different models of combining mental health and primary care To ultimately improve the health systems that support those with mental health concerns and those that care about them

Launching our project July August September October Developmental interviews with community members Received feedback from CARDS group on survey, videos, and usability United Way Forum Collect community opinions via survey Wrote and vetted video scripts with health systems Disseminate survey via community partners, WORT, and press releases Revised survey and methods of distribution Filmed videos with professional film crew and actors Further survey question development

How we’ll use the information To provide this information back as groups continue to work to improve care To conduct further research as to what models of integration “work” and for whom

What do we mean by “what models work”? Outcomes important to patients with mental health concerns and their families Outcomes important to those who deliver care in health systems and the community Outcomes important to those committed to the health, education, and safety of the public

Incentives to participate Include the voices of your organization and those it serves in improving care Keep the tablet or receive $250 for your organization Help the 1 in 4 people of Dane County who are affected by mental illness
