WHAT IS THE PURPOSE OF PUBLIC HEALTH OUTREACH IN COMMUNITY? Promoting Healthy Practices: Monthly Health Screens Preventing Disease and Injury: Providing Mass Immunization Clinics Advancing the Health of the Community: Educating Community with flyers, posters, handouts, & presentations The Outreach Department received 100% on the state Community Health Services Audit this spring, and were complemented on program organization and management.

WHAT DOES OUTREACH DO FOR THE COMMUNITY? Lots! IN-HOUSE RETINAL SCANS HEALTH SCREENINGS MONTHLY with COMMODITIES DISTRIBUTION CAR SEAT CLASSES for FAMILIES DIABETES WELLNESS SUPPORT GROUP DIABETES CASE MANAGEMENT HOME VISIT PROGRAM WELLNESS GARDEN EMERGENCY PREPAREDNESS FITNESS AND WELLNESS CENTER with PERSONAL TRAINER MENs WELLNESS AND SUPPORT GROUP

HOME VISIT PROGRAM A home visit may include weekly blood pressure and blood sugar checks, community resource assistance, Diabetes education, nutrition counseling, transport to a medical appointment, or medication pill-box set-up. We see 33 patients regularly each week by Outreach Team, the majority have chronic disease and/or Diabetes.

WELLNESS GARDEN A Medicine Wheel Garden was planted near Community Wellness Center. Herbs, fruits, and vegetables were plentiful this summer. We planted the garden to symbolize the Native Medicine Wheeloffering balance to the community. Many elders received garden veggies over the summer it was not much but-- greatly appreciated by all. The four spheres represent: Mental Spiritual Physical Emotional Health

FITNESS AND WELLNESS CENTER Johnny Gonzales operates the gym, and serves as the Fitness Trainer. He provides 1:1 fitness and nutritional education to Native American patients. 27 people utilize the gym on a regular basis, from ages The California Indian Health Services Diabetes Director complemented Lake County Tribal Health on our Community Fitness and Wellness initiative and will be sending a representative from NATIVE LIFE magazine, who will be writing an article for the January 2010 publication.

FITNESS AND WELLNESS CENTER WELLNESS CENTER: WEIGHT LOSS Patient A: Sept-March months Loss of 36 pounds, 6 inch loss/abdomen Patient B: Sept-March months Loss of 33 pounds, 6 inch loss/abdomen Patient C: February-April months Loss of 20 pounds, 2 inch loss/abdomen

RETINAL SCANS Retinal Scans performed by Outreach staff, photo forwarded to a UC Davis physician for interpretation. When we began program in early February only 12% of Diabetic population had received eye screens. We are now at 62%--many have received cataract/laser surgery, reducing the need for eye glasses. Diabetic patients have been our target, over half of those screened at LCTHC have been diagnosed with Diabetic Retinopathy, Macular Edema, Glaucoma, or Cataracts.

CAR SEAT & SAFETY EDUCATION Ferlun/CHR provides car seat safety class, the 2 nd and 4 th Thursday of each month. Bike Helmets and Smoke Alarms are also available.

DIABETES WELLNESS SUPPORT GROUP Provide Diabetes Education for the adult population over 50, twice a month. Outreach Staff to provide a multifaceted approach to decreasing the complications of Diabetes by offering: Traditional Cooking Class Preparing Healthy Quick Bites Chair-Robics Tai Chi for Beginners Understanding your Medications sessions Diabetes Management sessions, quarterly doctor appointments Emergency Preparedness for elders, and Fall Prevention Tips

DIABETES CASE MANAGEMENT Joyce/Diabetes Prevention Assistant sends out reminders to those in the community needing diabetes management labs, eye appointments, foot screenings, etc. Performs monthly audits, analyzes diabetes statistics and outcomes for the community. Work closely with Medical Team.

EMERGENCY PREPAREDNESS Tom/CHR and Disaster Preparedness Coordinator attends local and state emergency preparedness meetings. Attends Pandemic Flu and other Emergency Preparedness/Communication Drills, representing the tribal community. Procures/maintains emergency/disaster supplies for the community Prepared mass flu clinics for all rancherias this fall and a successful drive-thru at LCTHC.

COMMUNITY EVENTS UC BERKELEY EYE SCREENING AND EYE GLASS CLINIC- Spring A collaboration with Lake County Tribal Health Outreach Department and Sue Feliz from Scotts Valley. Over 75 people received eye screening from Berkeley Team. JUST MOVE IT- May through September 2009 A Community Walking Program, encouraging fitness and good health. Participants received t-shirts, water bottles, and pedometers. Prizes awarded to top-walkers with most mileage. TCOYD CONFERENCE- June 2009 Members from the community attended Native Diabetes conference with Outreach Department staff. Meals, registration, and transportation provided location was in Sacramento. TRIBAL OLYMPICS- July 2009 An excellent program for young children to exercise in friendly competition with local rancherias. A huge event offering good food, gathering of tribes, a fun day for the family! Over 300 gathered this year for event. TULE BOAT RACES- July 2009 Outreach staff provide blood pressure and blood sugar screenings, health education, and a healthy snack for 2 day event. FLU CLINICS- Fall 2009 Each year flu clinics are offered at each rancheria, in addition to flu shots for the elderly/disabled per home visit. Over 350 were immunized in September. Plus, we launched a successful Drive Thru Flu Clinic.

SUMMARY The Outreach Department works to support the mission statement of Lake County Tribal Health by: Improving the physical, spiritual, emotional and social status of Native Americans through culturally sensitive health care services.
