Community Practice Residency Advanced training opportunity with direct patient contact to enhance clinical skills, develop and implement patient care services, to become leader and innovator in community practice pharmacy and optimize transition of care. Purpose and goal. Develop our RPh for specialty therapy management.

PGY1 Community Practice Residencies Nationally 88 accredited community practice residency sites in US 6 in Ohio ONU, OSU, UT, Cincinnati Health Partners of Western Ohio Nationwide Children’s Hospital None in Northeast Ohio Data from ASHP

Program Requirements Manage and improved medication-use process Provide evidence-based, patient-centered care and collaborate with other health care professionals to optimize patient care Exercise leadership and practice management skills Demonstrate project management skills Provide medication and practice-related information, education, and/or training Utilize medical informatics

Overview Meet residency requirements for PGY1 Community Pharmacy Residency Program Fulfill staffing responsibilities while completing residency Complete teaching experiences through collaborative relationship with NEOMED Provide MTM service, adherence monitoring, wellness and preventative care.
