Community Practice Residency Advanced training opportunity with direct patient contact to enhance clinical skills, develop and implement patient care services, to become leader and innovator in community practice pharmacy and optimize transition of care. Purpose and goal. Develop our RPh for specialty therapy management.

Staffing Component 1 day per week in outpatient pharmacy Initial training required Weekend rotation in JJ Pharmacy 1 day per week in anticoagulation clinic 1.5 day per week providing scheduled MTM service

Projects Drug utilization evaluation for Employee Health Plan P&T Committee One in Fall Operational Expand on current Medicare Part D MTM service, Caresource MTM, and EHP Design, recommend, monitor, evaluate patient-specific therapy for selected disease state (diabetes, asthma, hypertension) Ohio – 1st state with Medicaid reimbursed MTM service Ex: newly diagnosed diabetics from EHP, educate on diet & exercise with therapy management (wellness) Medication Use Evaluation project can be presentation at midyear
