Purpose of Call To review revisions made to the IFSP form Revised form to be used on July 1, 2015 Changes based on feedback from CCBs, and Rule and procedural changes to the FMAP Today we will be reviewing the revisions to the current IFSP form that will be in effect on July 1, 2015. The changes are based on feedback received from service coordinators and EI Directors throughout the year as well as Rule changes that are reflected in the 2015-2016 State Plan and the Fiscal Management and Accountability Procedures.
Format of Review Each page of the IFSP with changes will be reviewed Changes are highlighted Revised form is appropriate for Infants AND Toddlers We will go through each page of the IFSP form that contains revisions. A complete copy of the revised IFSP form with changes highlighted is on the website under the TA Call materials for today. You can follow along with the call using this form. The revised form is appropriate for both Infants AND toddlers. Use of the IFSP with Infant Developmental Focus is no longer required.
Page 1 : Column One Transition Plan added. You would check this box if the IFSP being developed also contains the transition plan. Interim IFSP was moved to before Transition Plan, instead of being the first option Late, reason was removed . This will be documented in the data system, but isn’t necessary to be included in the family’s plan Up to 24 months was added to the adjusted age. Still get questions about the length of time the child’s age should be adjusted Other was removed and Home Visiting Programs was added. Column Two Best Method of Contact: , and Child’s County of Residence was added to the Parent Information section (Under 18) was added to the Child Lives With section. You should still note all the members of the family, but the age of adults in the household is not needed Service Coordinator was moved up to be the first contact information after the parent information School District/BOCES was changed to Administrative Unit(School District) to align with language in Rule Email was added to AU information
NOTE HERE that Page 2: “Daily Routines and Resources” or the Family Information page has been reordered and now falls after the evaluation and assessment page to reflect the requirement that family assessment be conducted with parental consent for all ELIGIBLE children Page 2 : Present Levels of Development Under General Health, Vision and Hearing Status If so, what is it?, changed to If yes, please note here: Under Vision and Hearing In the last six months added to clarify that the screening or testing must be recent Regarding follow-up added to note action needed if child has failed the screen or test Under Physical Development Descriptive wording changed from “movement and motor skills” , to “Body movements to change position or location such as rolling over, sitting, creeping, crawling, standing, walking and using hand for reaching, playing or eating” , to make it more family-friendly and incorporate language from IFSP with Infant Developmental Focus Headings for all Present Levels of Development Sections changed from Things I do well and Things I find difficult or need help with to: Things your child does well, and Things your child finds difficult. All IFSP plan language now talks about “your child” throughout the document. This language should help to support recording of the child’s participation within routines rather than recording bulleted skills
New Page 3: Under Communication Development Descriptive wording changed to “Interactions with others such as looking, listening, smiling, sharing emotions, using sounds, gestures and words to express thoughts and feelings, and understanding the words of caregivers.” , to make it more family-friendly and incorporate language from IFSP with Infant Developmental Focus. Under Cognitive Development Descriptive wording changed, to “Thinking, learning and reasoning, playing and attention to people, tasks and details. For babies, this may include falling asleep and waking up and finding voices and sounds in their surroundings.” , to make it more family-friendly and incorporate language from IFSP with Infant Developmental Focus
New Page 4: Under Social Emotional Development Descriptive wording changed to “Developing relationships, self-soothing strategies for becoming and remaining calm, getting along with others, and expressing feelings” , to make it more family-friendly and incorporate language from IFSP with Infant Developmental Focus. Under Adaptive Development Descriptive wording changed, to “Self-help skills such as eating, drinking and dressing. For babies, this may include reacting to their surrounding through changes in color, breathing, or digestion.” , to make it more family-friendly and incorporate language from IFSP with Infant Developmental Focus Additional child and family information Previous “Additional present levels of development and child and family information to be considered” from Present Levels of Development pages, and “Additional Activities” from Concerns and Priorities page have been consolidated here. This is where you should document any other information that is important to give a complete picture of the child and family, such as clinic-based services that the family is choosing to participate in or informal groups or activities, such as Mom/tot groups. This is really an “open” space for recording important information.
New Page 5 (previously Page 9): Evaluation or Assessment Results Methods and Procedures Required replaces Methods and Procedures Used Guidance language – “Check all that apply. Required if bolded. Additionally, the team must gather information using at least one other method or procedure” was removed. Methods and procedures are now delineated between those required for Evaluation and those required for Assessment and aligned with regulatory language. Some of these may be checked during the evaluation process and others during the assessment process. Evaluation or Assessment Results First two columns on old form “No Delay”, and <25%” have been combined and renamed , “Within typical range or less than 25%”. The option to note standard deviation was removed from the column heading, as they do not align with percentage of delay. If one of the evaluation tools being used provides a result that notes standard deviation, that should be noted in the Comments box Eligibility Determination Reworded to align with regulatory wording
New Page 6 & 7 : Information Gathered through Family Assessment – Entirely New Page Replaces Daily Routines and Resources Page This page is where the information gathered through the facilitation of a family assessment tool is recorded Wording that may be used to help explain the family assessment process to the family has been added Wording to explain the voluntary nature of the participation of the family in this process has been added (previously in the heading for this page) Check boxes for approved tool used and date tool was facilitated with the family Routine: document the time of day or routine that the family talked about What Does This Look Like?: Briefly describe the routine. Include enough detail to give a picture of this time of day, the child’s participation in the routine and the satisfaction of the family with the current state. May be expanded to include additional routines This page does not replace the protocol for the chosen approved family assessment tool. As evaluation results are noted within the present levels of development, results of the family assessment are noted here.
New Page 8: Concerns and Priorities Revised Concerns and Priorities/Additional Activities page The guidance wording “This may include difficulties with eating, sleeping, playing, communicating and behavior, etc” has been removed. This is where you should be recording the concerns gathered through family assessment and supporting the family in their prioritization. The Additional Activities section was removed and is now within the present levels of development, allowing more room for documentation on this page
New Plan of Action pages: Pages 9-13 Minor changes in wording “For your family” added to support keeping things family focused “We” changed to “you” in “what you see now and Outcome Statement “The” in the strategies heading replaced with “your”
New Page 14: Periodic Review or Annual Review of Outcomes, previously Review page New requirements due to needing to collect data on whether the individual outcomes on the IFSP are met. Each outcome will be reviewed at both the periodic IFSP review meeting and the annual IFSP meeting Outcome: Note wording from the outcome Progress: Note progress made toward outcome in narrative In right column, note either “Outcome met”, or “outcome not met” If outcome is not met, you can either end it there or choose continued, or revised Example: Continued – The child has not quite met the outcome, but it is still important to the family and they want to continue to work toward it as it is currently written Example: Revised – The outcome is still important to the family. The child has not met the outcome as it is too ambitious and is revised to better reflect an level of participation of the child that is accomplishable. Example: Met - The child has met the outcome. A new outcome needs to be written, even if the outcome is continuing in the same vein, such as working on a higher level of successful participation within the same routine.
New Page 15: IFSP Agreement Changes made to align with new definition section in State plan and the Fiscal Monitoring and Accountability Procedures Model - choose Primary Service Provider, Multidisciplinary Provider or Single Service Provider Method – Choose one : Individual, Co-Visit, Teaming, or Supervision . Choose one of these for each service documented. The methods of Teaming or Supervision are not stand-alone, but are always associated with another method of service. Frequency, Intensity, and Individual or Group are now in individual columns, to align w/ regulatory guidance. Funding Source: Other has been removed and CHP+ added to align w/data collection
