ECHO Colorado Extension for Community Health Outcomes

What is ECHO COLORADO? ECHO ~ Extension for Community Health Outcomes ~ is a knowledge sharing model. It is an innovative approach to building capacity by increasing participants’ access to expert and peer knowledge. The ECHO model is designed to… increase access to expert knowledge and support, reduce professional isolation, reduce high cost of off-site trainings, and create a community of learners.

Caring for Medically Complex Children Learning Objectives Better assess and manage the care for children with medical complexity. Create a shared care plan including all the different community and medical care providers involved in the care of a child with medical complexity. Better engage with other types of providers involved in the care of a child with medical complexity. Identify and share the resources available in their home communities for children with medical complexity.
