PHA Amendments – Extension to 2019 9/19/2018

Study Timeline – EXTENSION from 2017 to 2019 June 2019 subject recruitment ends Sept 2019 site closeout complete 2019 Hospital recruitment ends Subject recruitment continues 2018 Extend hospital contract to 2019 2017 9/19/2018

ACTION ITEM: MaRISS PHA Amendments Extend the study at your site from April 2017 through to 2019 Emails with the agreement amendments have been sent out to all eligible sites Required for each and every active MaRISS hospital, if you have not received this please let us know at mariss@heart.org Work with your legal/regulatory departments to complete this document soon as possible. IMPORTANT CHANGES: IRB reimbursements – please send invoices to mariss@heart.org within 90 days of your IRB approval date. ISC reimbursements – only for 1 person per site ISC 2017 registration receipt/confirmation is required in order for a site to receive reimbursement. Send documentation within 90 days of ISC 2017 to mariss@heart.org 9/19/2018

***New*** $50.00 MaRISS PRO Reimbursement As noted in PHA Amendment, for each patient that completes the MaRISS PRO online tool, an additional $50.00 will be paid to the hospital that consented that patient. Applies only to the first 100 patients that complete the PRO tool. 9/19/2018

Steps For Identifying MaRISS PRO Patients Day 1 - Day 3 Consent patient. Collect email address, contact info. Conduct MaRISS assessments. Confirm Day 30, Day 90 process with patient before discharge. **NEW** Day 90 Call During the Day 90 call, assess PRO eligibility for each patient. At the end of the call, ask if you can email them a link to answer the same questions they have completed on the phone. When patient agrees to PRO Log into the PRO tool and enter the patient’s MaRISS ID. Forward the survey link to the patient. Email MaRISS ID to mariss@heart.org Identify PRO Patient in PMT. Reminder: The survey is active for 8 days from the time the patient is entered into the HealthCall web portal. Please remember to remind the patient to complete the survey before it expires. 9/19/2018
