Project Summary Project Name: The Nile Corniche Project 2 Project Summary Project Name: The Nile Corniche Project Location: Egypt Start Date: August 1, 2010 Recent Manpower: 3,307 Scope of Work: Total built up area of 197,227 m2. Tower 1 comprises 102 residential apartments and 4 level penthouse. Tower 2 comprises 226 standard hotel rooms and 60 hotel suites as well as 98 hotel serviced apartments and 16 Luxury residential apartments. 7 story Podium (lobbies and QDE offices), over 4 level of car parking (667 spaces). Incident Details What Happened? Root Causes Direct Causes 3 On 28 December 2014, a High Potential Near Miss occurred when a forklift operator was attempting to erect a 10.5 meter steal column weighing 1.5 tons using a forklift. A General Foreman received a phone call to reinstate the Project fence line. These instructions came from the senior construction manager. The General Foreman went to the site and inspected the work area, he then called for a fork lift and assembled his crew. The fork lift drug the column out to a clear area and started to maneuver the column into place. The column was shifted on the fork lift in a horizontal position and eventually aligned with the base plate. At this time the General Foreman decided that a mobile crane was required to complete the job, told the crew to wait and left to make arrangements for the crane. The forklift operator then decided to attempt erecting the column. The forklift lifted the column once then set it down then lifted it a second time to about a 45 degree angle at which time the column slipped off the forklift and fell on the consultant site office that was fortunately not occupied at the time. Procedure - CCC FEL (Front End Loading) procedure was not followed, the work was not planned nor a mobile crane arranged. “No Risk Assessment/Method Statement/Job Safety Analysis/Job Safety Task Instructions/Toolbox talks). Work Direction – No JSTI perform by the task supervisor to identify task hazards. Work Direction – Task Supervisor failed to give proper instruction when he stopped the task to request a crane. Training – Task Supervisor was not trained on the required CCC JSTI process/procedure. Direct Causes Wrong equipment used (a forklift was used instead of a crane) Lack of communication where the task supervisor failed to give proper instruction when he stopped the task and left to request a crane Lack of competent supervision (supervisor is not trained) Inadequate HSE Training Poor decision making and judgment – disregarding safety rules Inadequate Hazard Identification (operator, employees and supervisor failed to recognize the unsafe act) In a hurry to complete a task. TAP Root Cause Analysis 6 TAP Root Cause Analysis Dec 28 Dec 2014 – 1100AM 14:30 PM 15:30 PM 16:00 PM 16:30PM 16:35 PM 16:55 PM Supervisor received instruction from CM-facades to reinstate a section of the projects fence line that had been removed to facilitate the removal of the project tower cranes. Supervisor reviewed the task work area. The forklift operator received instructions to assist in the preparation/erection of a 10.5 M column. The work crew started the task. Column slipped from the forks of the forklift Initial notification of the incident was given to the task Supervisor. HSE Dept. notified of the incident He requested the forklift & crew to come to the area. Forklift Operator & crew proceeded to the work task area. Mr. Hany received instruction from Supervisor to shift the column from the area where is was laid down to line it up with the base plate for erection. Supervisor informed of the incident by PMV Mgr. then trough to the PM Mgr. Project HSE Team started an investigation of the incident. The task was not planned for in the projects tower crane 3 removal plan (FEL). The column was being lifted by the forks of the forklift (steel on steel). The crew used 2, 3 M lifting slings to secure the column to the mast of the forklift. On the second attempt to raise the column, the column slipped from the forks of the forklift when the column was raised to about 45 degrees.. CF Supervisor stated that he did not think he needed to conduct a JSTI for the task. (Training). Following the notification of the incident, the incident scene was not protected and employees started to repair the damaged office and shift the column from where it had struck the office. A tower crane had originally removed the column and set in the storage location before the removal of the tower crane. The project has no hired mobile cranes for use at the time of the incident. (recourses) CF The crew used none color coded inspected rigging materials (Shackle and 2 slings). The crew took approximately 20 minutes to shift the column from the lay down point to the point of alignment with the base plate. It was observed by the Supervisor that a crane was needed. The Supervisor left the area for sunset prayers. The Supervisor did not give clear instructions on stopping the work, he only stated stop we need a crane. The crew continued to work, after the Supervisor left the area. No MS/JSA/RA/JSTI/TBT was conducted for this task (CCC Procedures). The incident area was requested to be secured and all work stopped by HSE team and CCC HSE Group. The incident scene was not protected. CF CF CF DC Lessons Learnt Consolidated Contractors Company
