Person centred planning (PCP) to develop their own identity Aspirations are at the centre of all decisions made All services coordinate with each other throughout the whole process A clear timeline of support that they are receiving and the outcomes that will be achieved Be aware of and build on the progress made by other teams Utilise and share the information about the child or young person
