Components of Person Centered Planning for High School & Transition Students Gain a basic understanding of transition planning and future directions A pre-planning meeting will help the student, family, and school decide the best circumstance and logistics for the planning meeting A team is developed around the student to assist in planning and implementation A Person Centered Planning meeting is facilitated by an outside facilitator such as a staff person from WiSe, or it may be an educator or employment provider Person Centered Employment Goals are aligned with IEP goals The school and adult service system work with the student and family to reach identified goals
