The PHP is Central to the Whole Health System

Personal Health Planning The personalized health planning process involves partnering with Veterans to achieve optimal health and well-being. This is a process of exploration, not of just the health issues of Veterans, but of their values and ideals.

Telling Others About Whole Health: Your Elevator Speech Passport, pg 21 Draft a thirty-second elevator speech You can use suggested elements from the next slide or make up your own Share your speech with your partner, and listen to his/hers Offer insightful feedback If time allows, share with your group claybanksstudio.com

Some Talking Points Is a new approach health care Looks at the whole person Respects the individual uniqueness Is about personalized, proactive, patient-driven care Gets people asking, “Why do I want my health? What really matters to me?” Aligns with VA strategic plan and patient centered care Brings in CIH approaches, when appropriate Is being adopted by many VA sites Focuses on clinician well-being too Involves creating a personal health plan for each Veteran Respects the importance of prevention Incorporates mindful awareness Emphasizes self care, to empower people to care for themselves Relies on teams, and the patient is the captain of his/her team Does not get rid of conventional care, especially for acute problems

Reviewing Your Partner’s PHI – Just Looking, Not Discussing Yet! NEW PARTNER: Pass your PHI over to your partner The PHI is a springboard into the conversation Follow a stepwise process: First Page: Look at Ratings (Physical/Emotional/Life) First Page: Read their answers to “The Big Questions” Second Page: “Where You Are & Where You Would Like to Be” What numbers stand out? Which ones are lowest and highest? What areas have the biggest gaps between the numbers? Example: Ask – “Why not a 2 as opposed to a 3?” Second Page: Look at the reflection questions
