Free template from www.brainybetty.com If the request for testing might include testing for vision, hearing, health, etc.  it would be a good idea to give an administrator a “heads up”.   In most instances when health is in question the parents will already have the documentation. Students with IEPs will use interventions but not the RtI process.  Regular education teachers who see areas of need not addressed in current IEP will use interventions, collect data and either share at the next IEP meeting, or send to the special education teacher and ask for an IEP review.  This process will not be an official RtI. 9/22/2018 Free template from www.brainybetty.com
