HGRH UNITS SCHRADER VALVE Issue description: Units from CMX-C  had Schrader cores that did not allow the TXV to operate. Containment at the factory made on August 28, 2013 Corrective action - Change assembly design, eliminate nut of the TXV Valve assembly and eliminate coremax on the tubing. Design change made to brazed in equalizer line. Units built after February 7th will have this change implemented. Clean Point SN C140689853

SRT and MRT Gas Valves Issue Description: Gas valves failing “stuck open” or “stuck closed” Root Cause: appears to be moisture infiltration causing “shorting” of the circuit board located under the cap of the gas valve. Preliminary findings show that on the SRT chassis 2 a plug is installed on the unit top. The plug is used to seal a screw hole that is used on chassis 1 units. This plug may be missing.

SRT and MRT Gas Valves Root cause and containment: SRT chassis 2 found in inventory with missing plug. SRT chassis 1 showing “cocked” (misaligned) screws in top Both occurrences are the leading cause of water infiltrating the burner section and causing the gas valve failures. Quality alert implemented at factory on January 18 2014 to warn the operator for correct assembly of plastic plug and screws on Top. Gas valves were added to the Special Request Parts return list for vendor analysis Nov. 4, 2014. Design team is looking for the best option to eliminate the water filtration. Validate new parts.

SRT and MRT Gas Valves Corrective actions: New seal strip in place New method for routing cables Changes implemented Week C1416

SRT and MRT WATER LEAKS Issue description: Water leaks through the hinged panels causing damage in the building. Containment: Factory Quality alert implemented on November 26, 2013 Root cause: Missing gasket in brackets Wrong position of gasket in unit Wrong position of aluminum plate on unit.

SRT and MRT WATER LEAKS Corrective actions: Factory modified operator standard work instructions and evaluate with Process check list to assure correct placement of gaskets Rain test validation in a Unit every 4 months. Corrective actions implemented on February 25, 2014

CMX-C Water Leaks SRT,MRT Units Issue description: Units are leaking water into the return air section Containment: Ensure silicone application in the bottom of the evaporator coil. Ensure silicone application in the holes between compressor plate and base pan. Root cause: Lack of or missing silicone Corrective actions: Apply seal strip instead of silicon between base pan assembly, design team is making a rain test for validation of this change. Changes are in process.

SRT UNITS WRONG LABELS APPLIED Issue description: Label 208V-230V placed in 460V units . Root cause: Manufacturing operator error due to standard work instructions not being clear when “not to use” the label. Corrective actions: Proper training of staff on modified standard work instructions. Add this Label to the unit BOM. Changes implemented on February 25, 2014

CMX-C Flame Retainer Issue description: Burner with Flame retainer incorrect. 2 Holes instead of 3.Sensors on burner were damaged due excessive heat. Containment: Operator is validating with a mark the correct Retainer assembled on unit. Quality alert issued at the factory

CMX-C Flame Retainer Root cause: Corrective actions: Design Issue allows for use of the wrong retainer. Corrective actions: Mfg. is working with the flame retainer die to find the best position of screw holes in each part number. modify the vestibule plate to match with new retainers. Target for April1,2014 Setup selection for press shop will be monitored by checklist. And go no go fixture. Target is May 1, 2014

TITLE 24 COMPLIANCE Title 24 is a California Building Energy Efficiency Standards Code. For ICP, Title 24 compliance pertains to the efficient design and operation of HVAC equipment. Applicable equipment that is to be sold in California in July2014 must comply with this standard. Specific Title 24 compliant functions include: Dual Minimum Economizer Positions, based on fan speed/airflow Pre-Occupancy Purge Demand Control Ventilation Occupancy Sensor Ventilation/Setback, which widens the occupied set points when the space is unoccupied Heat Pump Aux Heat Control, which only allows auxiliary heating operation when it is necessary Economizer Fault Detection and Diagnostics Fault detection - performance based

Economizer (FDD) Fault Detection and Diagnostics FDD example:

Condensate Drain Pan Problem description: Drain pan breaks after installing tube due to dimensional problems with internal and external diameter of tubing hole Root cause: Uncontrolled process during production Corrective action: sub supplier to run the process with the correct cool down time, as well as implement audits to check the parameters and size of the internal diameter. Implemented week 0414

COMFORT ALERT DIAGNOSTICS MODULE Issue description: Comfort Alert Diagnostic Modules (CADM) are being misdiagnosed on system startups P/N 1179496 Products affected: CAS, CHS Returned CADM’s failure analysis report confirm no defect found (NDF)

RHS Heat Pump Factory Mis-wire First field report: RHS – week 08/2013 Found the violet wire connected to the ODF contactor coil is connected to CLO2 at pin #7 on the CTB board. This causes the condenser fans to continue to run during the defrost mode, thus resulting in partial icing of the coil
