State Health Improvement Successes Every Woman Connecticut Learning Collaborative Distribution of media campaign to target lead poisoning disparities Healthy Food Donation List and distribution to networks; approved by USDA and adopted by United Way of Greater New Haven Distributed HPV educational materials to networks to support HPV vaccine as a cancer preventive Collaboratively developed 2017 SHIP Policy Agenda Community water fluoridation statute updated to federal standards DPH receives accreditation designation; funding to support local health accreditation Use of networks to disseminate, educate and build momentum for legislation and other activities – HPV as a cancer preventive, CD – disseminating food donation list with roles for action teams and partner networks. Influence quality of food being donated. EH – media campaign, use partners to extend the reach Last year - - Legislation passed to adjust community water fluoridation standards Health improvement targets surpassed for reducing the prevalence rate of lead poisoning; media campaign to address disparities Every Woman CT pilot in 8 communities – preconception health counseling - improve birth outcomes and reduce unplanned pregnancy Documented successes in pre-exposure prophylaxis (HIV PrEP) as a preventative measure for persons engaging in high risk behaviors. 6 more LHDs are planning to apply for accreditation based on 2016 survey results of LHA CT communities not covered by CHNA – 14 in 2014 report. Proposed Concepts for 2018 Policy Agenda Comprehensive Tobacco including: Raising the age to purchase to 21 years Upgrade clean indoor air law to national standards Removing pre-emption clauses that hinder local control Seatbelts for all seating positions in automobiles Property Maintenance Code update Sugar Sweetened Beverages Last year, coordinated a Summit with Coalition members - identify a policy agenda that supported the priority objectives worked hard to focus on widest part of the pyramid to address Long lasting protective interventions Taxes/policies socioeconomic factors that have the largest impact. First time doing this as a coalition and we’ve had limited success Still waiting for budget to pass so outcome not totally known. For example: CD – obj/indicator – youth smoking – tobacco legislation Housing improvements ID – HPV/cancer prevention

Majority of CHIPS include a focus on Chronic Disease, health Systems and MHSA. For state/local CHIPs, there is general alignment among a subset of strategies within the focus area in Chronic Disease and MHSA, but not Health Systems where there is no alignment at the strategy level. Some CHNA/CHIP processes also identified MCH, Infectious (STI) and Injury/Violence. Hospital CHNAs – priority health needs selected – more than 20 of the 27 hospitals identifying CD (obesity, nutrition, respiratory health), MHSA, and Access to care.
