The first four pages will be the Table of Contents.

Page #5 should be folded in half.

On the last page, draw a diagonal line like this. You may use pencil for this part.

Cut along the line. You are making a pocket.

Cut the binding-side of the paper carefully. DO NOT TEAR THIS. I cut this about a quarter inch away from the seam

Now your page looks like this.

Glue along the two edges (left and bottom), but leave the diagonal open.

Finished? You should have four pages ready for Table of Contents You should have a back pocket. You can go ahead and start numbering pages  odd #s will be on the right, even #s will be on the left.
