Good things that have happened since the last review Matthew has started playing pool Has impeccable manners when out Qualified from the North Midlands in equal first place and took part in the National riding championships at Tetbury [came 4 th ] Slowly growing up, has his own views Good at listening Being invited to join a new youth club Learning Spanish and doing really well Making African pots in ceramics Good at money skills

Good things that have happened since last review contd… More adept at seeing the bigger picture – there is an end to school Going to interviews – at the George Hotel, Stamford Doing jobs, so that I can earn money for my holidays from dad Having a day out at Snibston discovery park with the youth club Had a very good report from my science teacher – tables, bar charts etc Making new friends outside of school Matthews Statement is working well, no changes are needed.

Questions to answer What do we have to do to get Matthew some individual guitar lessons? In maths, work on telling the time, adding coins, subtracting numbers below 10 mentally – How can Matthew improve on this? In English – how can we help Matthew further improve his reading and writing? How can we help Matthew improve his communication skills [speech and language]? How can we help Matthew be more aware of road safety and stranger danger?

How can we best support Matthew in relation to the six keys to citizenship in the future? FACE [Functional Analysis of Care Analysis] carried out at 17 by the social services department

Self Determination [advocacy, circle of support, communication] Support from connexions until 25 Leicester Advocacy Whatever Matthew is interested in will give him the self determination to succeed.
