Instruction / List the Dangerous Occurrence/Practices/Conditions observed: Emanating from the fatal accident and the observation conducted during the in loco investigation on the 17 January 2018. The following was observed: The now deceased was using the loader to remove the long anchor tensioner machine Lock out system on the mechanical loaders discipline not effective B. The instruction(s) and/or order given The employer is instructed to stop all long anchor installations at Tshepong Mine. The employer is further instructed to send all employees required to install long anchor installations and working on heights for retraining and the employer is further instructed to assess these employees with the help of a third party to ensure that the training is adequate. The employer is also instructed to ensure that all mechanical loaders is properly locked out, only employees trained and found competent are operating these equipment. COO must present this fatal to the Principal Inspector of Mines The employer is further instructed to, jointly with member(s) of health and safety committee or unions, fix the deviation(s), conduct a comprehensive audit for similar deviation(s), investigate reasons for system failure and institute an action plan to prevent further recurrence of system failures. This instruction shall remain in force until such time that the employer has complied with the instruction and a presentation made to the office of the Principal Inspector of Mines by the said employer and the member(s) of health and safety committee or unions.

Description of incident On 17 January 2018 at approximately 08H00 at 73 East North haulage, Mr. N Jacwana was injured when he apparently fell in-between the loader bucket and loader frame. Mr. Jacwana was using the loader to remove a long anchor tensioning machine from an anchor on the hang wall.

Sequence of Events A safety meeting was held at the waiting place with all crew members A discussion concerning safety declarations and safe work for the day was held The tasks that were planned for the day were discussed It was planned to go and fetch the long anchor tensioner from its position still on the hang wall in the North haulage The team leader Mr. P Monki sent two employees to retrieve the long anchor tensioner from the North haulage As Mr. Monki reached the accident scene he found Mr. Jacwana lying on the footwall in front of the loader. The crew members working in the 73 East North haulage heard Mr. Jacwana cry out while they worked at the face area ahead of the mechanical loader. The Team Leader known as Matjopi walked towards where the loader was situated , and was informed of the accident. When Mr. Matjopi reached the loader he saw that Mr. Jacwana was lying on top of the loader top deck 8. Mr Jacwane had a serious injury to his right leg and chest area. First aid was applied.

Findings by the Investigation Team The accident scene was barricaded off and the responsible Miner was standing guard at the entrance to the work area The following environmental conditions were determined: Wet bulb 27.2º C. Dry bulb 35.8º C. Velocity 0.8m/s. Noise 84.5 db A mechanical loader 380 SL 49 was found at the scene, blood covered to entire area of the top deck of the mechanical loader A disconnected 50mm air hose was lying in the development end lying towards the mechanical loader A long anchor pneumatic tensioning device was observed on the footwall next to the mechanical loader Air hoses and support jacks were observed lying on the footwall in near proximity to the mechanical loader Minimum water and mud was observed on the footwall An aluminium and wooden ladder was found in the development end A 25mm air hose was lying next to the tensioning device
