NURSES ARE BASICALLY INVESTIGATORS,USING ALL THEIR KNOWLEDGE AND RESOURCES TO PREVENT HARM AND PROMOTE WELL-BEING TO THEIR PATIENT WHAT? WHEN? WHY? WHO?

???? WHAT ???? To accurately stage pressure ulcers, and to know when ulcers should not be staged.

???? WHEN ???? On admission do a skin check and remove any dressings, including Duoderm. Whenever you change a Duoderm or pressure ulcer dressing.

???? WHY ???? Accurate assessment and documentation is critical in order to provide evidence of wound healing or failure to heal; effective communication regarding wound staging requires the use of accurate and universally- recognized terminology and descriptors.
