Measures FloridaHealthFinder.gov a service of the Agency for Health Care Administration Measures Measures Being Displayed on FloridaHealthFinder Period Performance for Medical Condition or Procedure (Adults) October 2016 through September 2017 Performance for Medical Condition or Procedure (Pediatrics) October 2012 through September 2015 Readmission for Medical Condition or Procedure (Adults) July 2016 through June 2017 Readmission for Medical Condition or Procedure (Pediatrics) July 2012 through June 2015 Cases by Physician (Hip and Knee Replacements only) Measures NOT Being Displayed on FloridaHealthFinder CMS cancelled the July 2017 update due to issues involving several quality measures, including central line-associated bloodstream infections and Clostridium difficile infections. The last time CMS updated these ratings was on December 21, 2017. Due to the change of utilizing ICD-10-CM codes for the last quarter of 2015, the Agency for Health Care Research and Quality (AHRQ) will NOT update measures that reflect risk adjustments until the new codes are collected over a sufficient time period. Hospice Quality Measures FloridaHealthFinder.gov a service of the Agency for Health Care Administration Hospice Quality Measures HIS and CAHPS® CMS has finalized the specific collection of data items that support the following 6 NQF-endorsed measures and 1 modified measure for hospice: NQF # 1617 Patients Treated with an Opioid who are Given a Bowel Regimen, NQF # 1634 Pain Screening, NQF # 1637 Pain Assessment, NQF # 1638 Dyspnea Treatment NQF # 1639 Dyspnea Screening, NQF # 1641 Treatment Preferences, NQF # 1647 Beliefs/Values Addressed (if desired by the patient) (modified). The Denominator – which is the number of total respondents to the question. Observed: Percentage/Hospice’s score. For each hospice, each quarter is weighted according to the number of decedents/caregivers responding for that score. Surveys utilized: A minimum of 30 completed questionnaires during the reporting period. The data set contains the national average (mean) “top-box” scores of Medicare-certified hospice providers. Top-box scores show the percent of respondents who gave the most positive responses for each measure. The higher percentage is better.
