It is where you specify what you expect in your medical treatment as well as what you do not want as a part of your medical treatment. This is an important document because it will govern your healthcare and your healthcare options.

HEALTH INFORMATION AND PATIENT PROTECTION ACT HIPPA Authority This gives your physician and medical facility your permission to describe your medical condition , either in person or on the telephone, to others you identify. Without this document, not even your spouse, parents or significant others will be able to get information about your healthcare. Local and federal government will usually take a portion. Your children will usually be remembered—equally. The organizations you belong to and institutions you serve will not be included unless you specifically write it down. This is included in your last will and testament if you choose.
