MEDICATION THERAPY MANAGEMENT SERVICES Getting the most from your medicine.

Medication Therapy Management Services A new Patient-centered service that: -Helps you get the results you want from your medications -Answers questions about medications -Provides a complete medication list -Shows how &why you are taking medications -Helps communicate with your doctors

Your Pharmacist Will: Meet with you face-to-face, uninterrupted, in a private setting, to discuss all of your medications, including prescription medication, nonprescription products, vitamins, herbal supplements, and nutritional products.

Your Pharmacist Will: Review all medications to make sure they are appropriate for you, effective for your medical condition, safe for you, and convenient for taking properly. Work with your doctors to make sure everything is done to meet your needs and improve your health

How Will YOU Benefit Patients who receive this service have more positive medication experiences and better results Doctors support the recommendations made by our pharmacists This service can save money, improve the clinical outcomes from your medications, prevent medication problems, and make you feel better

Paying for the Service In many states Medicaid patients can receive the service without a co-pay, as can Medicare patients insured through Part D plans Some private pay insurers may pay for this service This service is available to all others for a small fee Ask your pharmacist for details

Call for an Appointment (phone number here) Your first appointment will take about 30-45 minutes to review all of your medications and health questions Follow-up appointments will likely take 15-30 minutes Bring all your questions, concerns, and issues about your medications Bring in all medications (prescription, nonprescription, herbal remedies, food supplements, vitamins) You will talk privately about your medications The pharmacist will communicate with your doctors as necessary.
