Case Management: Assessing Recovery and Applying for Recertification Tom Faulkner, MD Senior AME, HIMS-IMS 25 April, 2018
The Pilot in Recovery Perspective “Right now” 4 months ago
AME/IMS Perspective Supported by objective documentation Along with the“gut” check (subjective) A unanimous team decision
Required Documentation All inpatient treatment records Aftercare monitors reports Sponsor letter Peer Monitor reports Chief Pilot reports Random testing results
Other certification documents Medical Conditions identified in treatment, SIA/AASI/CACI Administrative/Legal Arrest and court records Personal Statement by Pilot* * My requirement
Favorable Objective Data Contributing at AA, Aftercare, and Employer meetings, Open and frequent communication, Above and beyond work Appreciation above urgency
Quality of Involvement Status of personal recovery Step work Roadblocks Help with problems Group work Contributing to group Challenging others and accepting perspectives Lessons learned
Control vs. Cooperation “Checking boxes” Struggling with loss of control “He got back . . . Honesty, Openness, Admission of responsibility, Accountability Seeking and accepting help
AME/IMS Resources The more eyes watching, the better: Aftercare, sponsors, employer, other Pilots The Psychological & Psychiatric (P&P) evaluation Friends and Family
Promising behaviors Quick identification and treatment No “Voluntolds” Understanding of the impact it had on health, family, occupation, safety Planning For next week, month, year Return to work - layovers Relationships Progress not perfection
Warning signs “I’m not like them.” Limited exposure to other recovery groups Meeting the minimums. “How many meetings do I have to attend once I’m re-certified?” “Can I drink again after the SIA is removed?” “I do not want anyone to know about this.” WHERE ARE YOU?
Putting it in Writing HIMS Independent Medical Sponsor (IMS) Evaluation and Recommendations: I agree to serve as Mr. John P. Flyguy’s Independent Medical Sponsor (IMS) in his efforts to seek/maintain re-certification of his First Class Medical certificate under the HIMS-Substance Abuse Special Issuance Authorization program. It is my recommendation that Mr. Flyguy be granted a Special Issuance Authorization (SIA) of his First Class Medical certificate under the HIMS-Substance Abuse program.
Final Thoughts Recovery offers a rare opportunity to re-boot, make the most of it, Counseling and the FAA Medical, FAA stressing objectivity (vs. being advocates, Resources for family
