Figure 1 a–d. Case report. A 63-year-old male with severe Bechterew's disease and pancolitis ulcerosa had a total proctocolectomy and a terminal ileostomy in 1998. In 2008, he developed a parastomal hernia with sub-obstruction. In 2009, a perforation of the small bowel occurred and led to Klebsiella sepsis. Small bowel fistulas and peritonitis required multiple re-look laparotomies and a cool-down period of 6 months, followed by a partial enterectomy. Before, he practised Nordic walking to keep his muscles flexible in context of the Bechterew's disease. a. Midline dehiscence and fistulisation resulted in a contaminated transmural abdominal wall defect consisting of zones IA and IB. The colostomy was positioned at the left lateral side and the urostomy at the right side of the abdominal wall. b. Cartoon showing the PIVA harvest in relation to the defect. c. The perforators and the descending branch are dissected up to the origin at the deep femoral vascular pedicle. The PIVA flap is tunnelled under the rectus femoris and sartorius muscles to medialise the neurovascular pedicle and transferred to the abdominal wall defect to restore all layers. The proximal fasciocutaneous segment of the flap covered the vascular pedicle in the scarred groin area. d. After 12 months, the PIVA flap healed well and restored the integrity of the transmural defect with like-with-like tissues. The patient had no subjective loss of function at the donor thigh, even when mountain hiking and despite Bechterew's disease. The defect with exposed bowels is located in zone IA and IB. c The PIVA flap is tunnelled medially from the rectus femoris and sartorius muscles and rotated to the defect on the abdomen. Veins encountered posteriorly are clipped to avoid kinking and torsion of the vascular pedicle. d Postoperative result after 12 months. Strength at the donor site only restored after 9 months.
