Re-enroll if you need to: Add, cancel or change your health, dental or vision plan Add or remove a spouse, sponsored dependent, or dependent child Contribute to a flexible spending account Add or change the amount of life or AD&D insurance Enroll in or increase short-term disability insurance

If you take no action Benefit plan What you’ll have July 1 Health plan (HMO, PPO, EPO, RHP) Dental plan (UK or Delta Dental) Vision Voluntary life insurance Accidental death & dismemberment Flexible spending account The same plan you had for 2017-18 No money will be deposited

Gather documentation You will be asked to provide the following documentation to add family members to your plan: Spouse: marriage certificate and front page of tax return Adult sponsored dependent: documentation of one-year common residency Child: birth certificate, adoption, guardianship, court order, front page of tax return
