Continuum of Care: Structure Collaborative Applicant HMIS Lead Board 2

Responsibilities of the CoC: Operate the CoC Hold meetings of the FULL membership, with published agendas, at least semi-annually Make an invitation for new members to join publicly available within the geographic area, at least annually Adopt and follow a written process to select a Board to act on behalf of the CoC Develop, follow, and update at least annually a governance charter

Responsibilities of the CoC: Operate the CoC Performance Measurement Establish performance targets appropriate for population and program type in consultation with recipients and subrecipients Monitor recipient and subrecipient performance, evaluate outcomes, and take action against poor performance

Responsibilities of the CoC: Operate the CoC Performance Measurement Evaluate outcomes of projects funded under the ESG Program and the CoC Program, and report to HUD

Responsibilities of the CoC: Operate the CoC Establish and operate a coordinated entry system for the geographic area. CoCs should consider: Geographic size Provider relationships Other systems for specific homeless subpopulations already in place The CoC’s experience with HMIS

Responsibilities of the CoC: Operate the CoC Establish and follow written standards for providing assistance within the geographic area in consultation with recipients of ESG Program funds within the geographic area

Responsibilities of the CoC: HMIS The CoC must designate a single HMIS for the geographic area Designate an eligible applicant as the HMIS Lead Review, revise, and approve a privacy plan, security plan, and data quality plan for the HMIS

Responsibilities of the CoC: CoC Planning CoC must develop a plan that includes: Coordinating the implementation of a housing and service system that meets the needs of homeless households encompassing – Outreach, engagement, and assessment Shelter, housing, and supportive services Prevention strategies

Responsibilities of the CoC: CoC Planning CoC must develop a plan that includes (cont): Planning for and conducting, at least biennially, a PIT Count of sheltered and unsheltered homeless households Conducting an annual gaps analysis of housing and service needs within geographic area

Responsibilities of the CoC: CoC Planning CoC must develop a plan that includes (cont): Providing information required to complete the Consolidated Plan(s) Consulting with ESG recipients within the geographic area on a plan to allocate ESG funds and evaluate performance of ESG recipients and subrecipients

Preparing an Application for Funds The CoC must: Design, operate, and follow a collaborative process for the development of applications Approve the the submission of applications in response to the NOFA Establish priorities for funding projects in the geographic area
