Individual Dental Insurance For individuals and families who lack dental coverage through an employer We use one of the largest Preferred Provider Organization (PPO) networks in the Pacific Northwest LifeMap offers three individual dental plans: Dollar Based Dental Incentive 10 Dental EPO Dental through Willamette Dental Group Includes option to purchase Vison benefit rider Note: LifeMap Individual Dental does not meet the pediatric mandate for the ACA Change sub-head to: For individuals and families who lack dental coverage through an employer- done Please use the Next button to advance

Places dental care back in hands of the patient and the provider. Very few exclusions (cosmetic and orthodontic only) No age restrictions and pairs well with Medicare Available in Oregon, Washington, Idaho, and Utah.
