Never forget where you’re coming from “If you don’t know your history, you’re like a leaf that doesn’t know it’s part of a tree” Project 81 - a group of residents. including John Evans who lived at Le Court Cheshire home in Hampshire in the late 1970s. What is personalised care? People having choice and control over decisions that affect their own health and wellbeing within a system that harnesses the expertise, capacity and potential of people, families and communities in delivering better outcomes and reducing health inequalities. This incorporates: . a proactive and universal offer of support to people with long term physical and mental health conditions to build knowledge, skills and confidence through supported self-care and community-centred approaches whole population approaches to supporting people of all ages, and their carers, to manage their physical and mental health and wellbeing and make informed choices and decisions when their health changes Intensive approaches to empowering people with more complex needs to have greater choice and control over the care they receive

An integrated, whole population approach to personalised care

Key features of personalised care for people Personalised care begins with the belief that people are the best integrators of care, rather than other forms of integration. The key features set out what someone can expect and experience when personalised care is implemented well Everyone should:  Have access to a range of support options including peer support and community based resources to help build knowledge, skills and confidence to manage their health and wellbeing. Have the chance to formally and informally review their care plan. Be seen as a whole person within the context of their whole life, valuing their skills, strengths and experience and important relationships. If this leads to a personal budget, integrated personal budget or personal health budget, a person will:  Experience a coordinated approach that is transparent and empowering. Experience hope and feel confident that the care and support they receive will deliver what matters most to them. Get an upfront indication of how much money they have available for healthcare and support People who have a personalised care and support plan should:  Have enough money in the budget to meet the health and wellbeing needs and outcomes* agreed in the personalised care and support plan Be able to access information and advice that is clear, timely and meets their individual information needs and preferences. Be central in developing their personalised care and support plan and agree who is involved. Have the time and support to develop their plan in a safe and reflective space. Have the option to manage the money as a direct payment, a notional budget, a third party budget or a mix of these approaches Be listened to and understood in a way that builds trusting and effective relationships with people. Feel prepared, know what to expect and be ready to engage in planning supported by a single, named coordinator Be valued as an active participant in conversations and decisions about their health and well being. Be able to use the money to meet their outcomes in ways and at times that make sense to them, as agreed in their personalised care and support plan. Be supported to understand their care, support and treatment options and, where relevant, to set and achieve their goals Be able to agree the health and wellbeing outcomes* they want to achieve, in dialogue with the relevant health, education and social care professionals. * and learning outcomes for children and young people with education, health and care plans. 8

Personal Health Budgets Further diversification Headline figures Personal Health Budgets Further diversification IPC Programme Neuro disability (testing the IPC model for Multiple Sclerosis and similar conditions) 39,338 had some form of IPC personalised care plan at the end of Q1 2017/18 22,895 PHBs at the end of Dec 2017 This is a 75% increase on previous year 13,729 had an integrated personal budget at the end of Dec 2017 Substance misuse (3 sites testing IPC model)

Examples of progress WARRINGTON FYLDE COAST STOCKTON-ON-TEES Across Continuing Health Care, direct savings of c.17% (preliminary analysis); indirect savings of £4k per person WARRINGTON FYLDE COAST STOCKTON-ON-TEES NOTTINGHAMSHIRE Personal health budgets in end of life care - 83% were able to die in a place of their choosing, against an average of 26% 85% improved or maintained level of patient activation (average shift of 9.7 points), with attendant decrease in likelihood of hospital admission Significant reduction in unplanned hospital admissions for older people with frailty £19,000 saving in transport costs for siblings with very complex health conditions 35% reduction in delayed transfers of care from hospital Lease their own adapted vehicle through a personal health budget for journeys to day centre and respite, instead of a commissioned transport package One week’s worth of traditional services funds six weeks of services commissioned through a personal health budget 41% reduction in A&E attendances in the final quarter

The Secretary of State delivered a speech on 20 March 2018 outlining the seven key principles that will guide the Government's thinking ahead of the social care green paper, to be published later in 2018. …full integration of health and social care centred around the person. …today I can announce new pilots in Gloucestershire, Lincolnshire and Nottinghamshire which will mean that over the next 2 years every single person accessing adult social care will be given a joint health and social care assessment and - critically - a joint health and care and support plan, where needed. …I want to turbo-charge progress on integrated health and care budgets, making them the norm and not the exception when people need ongoing support. …today I can announce that we will be consulting on Personal Health Budgets, in order to achieve better integration for those with the greatest ongoing social care needs as well as health needs. …over the next 2 years in Gloucestershire, Lincolnshire and Nottinghamshire - our 3 pilot areas - every single person with a joint care plan will also be offered an integrated health and care personal budget.
