Purpose Inform/provide TSM healthcare and mental health care options to them and their family.

Standard Provide the TSM with a physical/dental check- up JAX/Norfolk Standard Provide the TSM with a physical/dental check- up Provide family copies of health records Separation History & Physical Examination (SHPE) The SHPE is a requirement for all separating members. For members making disability claim w/ the VA, the exam requirement may be met by the disability exam performed by the VA. If member wishes to utilize option, they should begin 180 days prior to separation.

DOD Mental Health Self-Assessment Program A Mental health and alcohol screening and referral program for service members and families. (affected by deployment and mobilization). Active duty may self refer at MTF A voluntary and anonymous program offered online, phone or through events on location. Self-assessments available for depression, bipolar disorder, alcohol use, (PTSD) and generalized anxiety. Individualized results and military resources are provided at the end of an assessment.

Services/Treatment Provided Psychiatric hospitalization Substance Abuse Disorder (SUD) inpatient/residential detox rehab Partial Hospitalization Intensive outpatient treatment Outpatient therapy/medicated assist Service offers Clinical/non-clinical prevention, education and treatment programs for active duty, Guard and reserve components. For additional care or support for these contact your regional Tricare or MTF.

Veteran’s Affair Coverage Eligibility Combat Veteran Authority Activated Reservist or Guard ID’d conditions in SHPE Veterans discharged from Active duty on or after 28JAN2003 provided enhanced enrollment placement for 5 years after the date they leave service. Enhanced enrollment available also to Reserve/Guard if on active duty of combat ops 11NOV1998 and left service under any conditions other than dishonorable. Separation History and Physical Examination, may make you elgibile for care in the VA after separation. Ensure to file/make claim with VA
