How to Access Data Collection Tools Login to the project website: https://www.safetyprogram4antibioticstewardship.org/ If you have not yet received login credentials, email antibioticsafety@norc.org with your first name, last name, and email address.
After you login, click on “Data Collection Tool”
Select “Type” and then “Collections" First select “Type” from the drop down menu. The following items can be chosen from the drop down menu. Once you select “Type”, only tools associated to that type of user will be visible in the “Collections” drop down menu. The following tools are available in the “Collections” drop down menu. After Selecting Type & Collections a List of Tools Will Appear “Status” tells you if the tool is Open, Closed, or has been Submitted. Click on “Open” to start the tool.
Common Features of the Tools Dates the tool will be open for. You can only complete the tools within a certain timeframe. Instructions page: Read the instructions to every tool carefully before beginning the tool. Progress bar to show % of tool completed. As you fill out each page of questions, click on “Save & Continue” to go to next page. Click on back to go back to the previous page. Questions will be listed here
Common Features of the Tools After 25 minutes of inactivity, the website will log you out. Make sure to save as you complete the tools. At any time you can click on “Save & Exit” to save the questions you have completed, exit the tool, and complete the rest of the tool at another time. Click on “Save & Exit” at the bottom of the tool.

Common Features of the Tools When you have completed a tool, you have the following options: Back: Go back to previous pages. Review: Review all of your answers on one page. Submit: Submit your answers. Once you click on submit, you will not be able to change your answers.
