Evacuation Procedures In the event of fire, hazardous materials release or other hazardous situations requiring emergency response the buildings may need to be evacuated. In the event of an alarm event: Exit the building through the nearest exit Do not stop to collect personal belongings Do not use elevators during an emergency event Go to the nearest gathering area and await instructions Alarm activation and notification To activate the alarm locate a pull station and pull lever. Stations are located at exit doors. The Alarm is a “loud” horn accompanied by a flashing strobe

Agoura Hills Evacuation Notes Unique Evacuation Info Assembly Areas are located at upper deck parking level (closest to Agoura Road) For Earthquakes, stay put & take cover immediately (desks & tables make good cover, doorways do not) When the shaking stops, evacuate to assembly area if you hear the building alarm Alarm Activation Activate a fire alarm pull station located at exit doors Alarm Notification Audible alarm is a loud Ringing Bell

First Aid/AEDs First aid supplies are located through the building AED’s are located in the lobby 1st floor, East atrium 1st floor, West atrium 2nd floor, and right outside the gym 2nd floor.

Illness/Injuries For emergency help, dial the 911and provide: Name, Location, & Nature of Emergency. It is extremely important that all work related injuries and illness are reported to your supervisor, even ergonomic issues.

Chemicals Chemicals must be approved prior to entering the Agoura Hills site by EHS A safety data sheet must be provided If approved, a copy of the safety data sheet will be placed on Teradyne’s online chemical management database Chemical Safety Sheets are available through the internal EHS website In the event the system is unavailable contact EHS. Backup CDs are available.

Hazard Assessments Hazard Assessments have been conducted to determine appropriate PPE for a job/area. PPE Protects “YOU”! Electrical Safe Work Practices While it is every employees responsibility to work and act in a safe manner, there are certain locations in the Agoura Hills facility that require additional electrical safe work practices and awareness. To access these areas you will be required to take additional training. These areas generally include all engineering labs and operational floors. Examples include but are not limited to:

Electrical Safe Work Practices If you enter these areas you should be aware of any potential hazards and receive electrical safe work practice training. If you have not received electrical safe work practice training then you should not enter these areas unless accompanied by someone who has and/or unless you stay in designated safe areas.
