Who Are We? Mission Support & Test Services, LLC We support NNSA as the M&O contractor for the Nevada National Security Site Transition from NSTec to MSTS end of 2017. Stockpile science, High-hazard testing, Threat Detection, Waste Disposal, Remote Sensing/Aerial Measurement, First Responder Training, Custom Fabrication, Groundwater Characterization, Research & Development

OPEX Program We use OPEXShare to distribute operating experience articles to the workforce MSTS 2,300 staff members (~1100 registered, 700 Non-bargaining employees) DOE Nevada Field office - 87 staff members (registered in OPEXShare) Daily ORPS summaries are shared through email and OPEXShare with managers.

Vulnerability Analysis U.S. DOE Office of Environmental Management, WIPP Accident Investigation Report (AIR) distributed throughout the Nevada Enterprise U1a Complex performed a vulnerability analysis based on the WIPP AIR Citations of identified weakness were extracted from the accident investigation report Analyst compared our Process and data to the identified weakness Risks or Program and Performance Vulnerabilities were identified Reference to existing issues or new issues were created to track mitigation strategies

Reviewing OPEX articles may provide an issue to enter into the Issues Management System to take preventive action Some of the shared Lessons Learned come from issues we have entered into the Issues Management System (caWeb) When it is uncertain how widespread an issue might be, we use an LL to share information with others When performing a Vulnerability Analysis on OPEX or DOE OE-1, or 2, we share in Issues Management items to track closure

Success Stories Reviewing Inventory Prevents Potential Laptop Battery Fire On October 12, 2017, a Safety Alert published by Pacific Northwest National Laboratory discussed that three Pacific Northwest National Laboratory (PNNL) staff members using Dell Latitude laptops (model #E7440) had discovered their batteries were “bulging.” After a review of the Mission Support & Test Services, LLC, inventory of Dell Latitude Laptops, Model E7440, three laptops of this model were located. One of the laptops batteries was immediately removed when a “bulging” battery was identified. All three laptops are no longer in service and were excessed.

Success Stories OPW Recalls Gas Station Hose Swivel Connectors Due to Fire, Explosion Hazards On June 07, 2017, a Product Recall Alert published in OPEXShare from the Consumer Product Safety Commission, OPW Recalls Gas Station Hose Swivel Connectors Due to Fire, Explosion Hazards.” This recall involved metal swivel connectors installed between the nozzle and hose of gas station pumps that dispense refined fuels such as gasoline, diesel, ethanol blends, and biodiesel blends. After a review of the Nevada National Security Site gas pump hoses, 7 OPW Hose Swivel Connectors were identified at the gas station. These parts were replaced.

Success Stories Elkay Recalls Water Cooler and Bottle Filling Stations Due to Shocking Hazards This recall involves Elkay and Halsey Taylor water coolers and bottle filling stations. Internal screws that secure the grounding wire can break and allow the grounding wire to become loose, posing a shock hazard. The firm has received 362 reports of broken screws being found in units prior to installation. No injuries have been reported. Consumers should immediately stop using the recalled water coolers and bottle filling stations, unplug the units and contact Elkay for a free repair. Recall date: January 25, 2018. One model unit was identified and tagged out of service and is waiting repairs.
