Care and support at home in Wales: 5 year strategic plan 2017-2022 ‘People in Wales want to live at home, even when they have complex, health, care and support needs.’ What we know: Tension between rising demand and available resources. Home care is often provided in isolation Home care is commissioned around an inflexible timetables; is overly complex and autocratic. Uneven relationship between commissioner, provider and service user. It takes: 370,000 unpaid carers (96% of care at home) Alongside: 214,317 hours of home care(£293m) to 39,500 people 4,463 people received Direct Payments

‘Your Choice’ A Pilot A simple, equal, flexible and person centred approach to commissioning care at home. A person with care and support needs and eligible personal outcomes. Single Care Agency (All Care Ltd) 15 people/325 hours per week Convert Care Timetable into a ‘bundle’ of care hours (+-8% variance) The person and the care agency are empowered to use the ‘bundle’ to meet eligible needs and outcomes in a manner that matters to the person and is achievable by the care agency. ‘Your Choice’ Aims and Conditions Based on Outcome-Focused Care & Support Plan Focused on eligible needs as identified in the Wellbeing Assessment Reduce administration burden Person-centred Self-directed through an equal relationship with the Care Provider Improve the lives of the people involved.

Your Choice

‘Your Choice’ Pearl’s perspective I am independent and like routine. I enjoy spending time with my family and talking with my carers. Through this project, Pearl has been able to use care hours more flexibly, meaning that when her daughter visits, she can ‘bank’ hours to use at other times – for example a political debate, or more time for social activities.

‘Your Choice’ Tony’s perspective I have many interests including rugby and vintage cars. I am chairman of the local MS Society branch and like to assist and advise people in similar situations to me. Through this project, Tony has been able to ‘bank’ care hours when representing the MS group for use with his other charitable interests and is planning to go to a Cardiff Blues match next season. He has also moved his hours around to have more time with the carers in the kitchen!

‘Your Choice’ Fred’s perspective l have a good sense of humour and enjoy a good chat. I have many interests including music concerts and aeroplanes as I was in the RAF for many years. Through the pilot Fred has been able to shift his care calls to allow him to attend the Veterans Club where he has been reacquainted with old colleagues and been able to spend quality time with his daughter.

Next Steps Roll out across all care agencies in the Vale of Glamorgan Each care agency have named Practitioner Manager/Senior Social Worker Each agency have a single ‘blended rate’ Each care agency work with people in an outcome focused way within agreed ‘care bundle’. Social worker to undertake 3 monthly ‘table-top’ monitoring and annual statutory reviews.
