Introduction Instructions at the beginning of the form indicate the different steps to follow in order to submit the application.

Introduction In order to submit the form successfully, you need to fill in each section. You can save your form at any time or quit/close the application without losing any of the information you would have already entered.

1- Company information This sections is asking you to provide with the details of the entity you are working under. Please provide with the name of your entity as well as with its postal address.

2- Request’s priority If your request is urgent, you will need to change the default value set as norma. Please note that should you change the priority to Urgent, you will not be able to pick your own PIN code and that you will need to validate the extra 30 euros charge with your hierarchy.

3- Information on the Recipient of the Card All mandatory fields need to be completed in order to successfully submit your request.

4- Information for individual payment The personal bank details are required to allow the Direct Debit set-up and are mandatory fields.

5- Method of Payment The payment method to be used is the Direct Debit. Information on this page are prepopulated when filling in the previous section.

6- Upload Supporting Documents You need to upload the below documents as a mandatory requirement. A copy (front and bacl) of your ID card or passport. Please ensure that they are valid. Your Bank Account Certificate (RIB)

7- Validate your request You need to validate the form by clicking on “Validate et Sign” in order to proceed with the eSignature of the form. You will received an automated email which will contain the secret code necessary to sign online.

8- eSignature You need to follow the below steps in order to sign online: Validate the Terms and Conditions Enter the secret code received via email. Click on “Sign and Submit” Once you have signed and submitted the form: You will received a confirmation email validating the request has been submitted. The authorized signatory will be informed that your request is pending for approval and will approve your request.

9- Follow your request Once the form has been completed and eSigned, you will received 2 emails automatically genereated and which will allow you to follow made on the creation of your card: - The first email will contain the access key The second email will contain the request reference number as well as the URL link

9- Follow your request Once your application has been approved by the authorised signatory, your request will be processed by American Express and the card will be send to you (under the conditions that your request is approved by the mandatory signatory and that the supporting documents are valid). These steps can be followed in real time.
