What is an exercise referral scheme? The aim is to provide an alternative to treating a patient by giving them a prescription for medication. The belief is that exercise can play a vital role in managing medical conditions and improving health. Therefore the medical professional (MP) will give them a prescription for exercise

All schemes will have variations but follow the same basic format of 10-12 weeks exercise taking place either individually or as a group Each patient will have to pay a small fee usually £1 per session, this is to make it available to all members of society and promote equality also by paying you are financially committing yourself to the programme

Who are the Medical Professionals Doctor (GP) Physiotherapist Occupational therapist Dietician Nurse Cardiac physiologist

Start of the programme MP will refer the patient to qualified trainer, who will then design a specific programme for that person However there are inclusion and exclusion criteria to be referred What do you think these are? Condition must be stable and have a proper diagnosis

How do we make the patient aware? How do we make the patient aware that a scheme is taking place Advertise in doctors surgeries Make sure that doctors know that they can do this Leaflets Open days at surgery/leisure centres Have this information available at hospitals and clinics

What information does the exercise instructor need? Age/sex/ characteristics of the person Past medical history and conditions and present status Medication Smoking/alcohol/BP/RHR Name, contact details Origin of referral Severity of the condition Have they previously exercised

Exercise Referral stages put these stages in order individually writing them into your books Return to the community Long term support to help the patient stay active Evaluation of the patient experience and health outcome Selection of the patient Physical assessment of the patient and the appropriate intervention applied
