Immediately escort student to CSC conducts Risk Assessment Procedures Suicide Risk Inquiry (Threat to Self: SUICIDE) Flow Chart for Certified School Counselor This work flow/procedure is for potential suicide ONLY. If you have a student that is at risk, but not suicidal, this is not the procedure to follow. Student is reported to school personnel for potential threat to self (suicide) Should the severity of the situation with the student call for an involuntary commitment, contact your school’s SRO or local law enforcement agency and notify parent.     Immediately escort student to Certified School Counselor (CSC) or School-based Social Worker (SW) CSC conducts Risk Assessment Procedures Student is at NO or LOW risk of self harm PHQ-9 or MFQ Screening Results Student IS AT RISK of self harm CSC contacts parent that concern was presented, risk assessment conducted, student did not appear at risk, Could offer Mental Health Services referral. Columbia- SSRS Results Student IS AT SERIOUS RISK of self harm Student is at ELEVATED risk of self harm CSC/SW notifies the principal. CSC /SW Consultation with school psychologist/ School Based Social Worker (LCSW). Principal notifies Office of Elementary or Secondary Leading and Learning. Parent is contacted for immediate action to mental health provider. Contact parent to encourage parent to discuss what transpired with student. Mental Health Services encouraged and/or referral is offered.   09-18 CE
