Health Plan Data Sharing with Health Home UHC provides the Health Homes with: A single point of contact (RN Program Manager) to work collaboratively to meet target goals of providing the six core services Daily Census Alerts based on Prior Authorization data on inpatient admission for care coordination (7 days follow up visits) Monthly Outcomes data that includes claims data on ER visits, PCP visits, Specialists and Pharmacy services Up-dated demographic information on members to assist CMA’s in successfully outreaching members to improve conversion rate Identify super utilizer members based on cost data and provide a roster to Health Homes Monthly JOC meetings with health homes to provide data on enrollment and claims data on Care Transitions Collaborating on the reporting of the other services rendered Proprietary Information of UnitedHealth Group.
